The following provides more detailed information on individual data items, most of which have already been mentioned in the OHID MDS - NOA Required data items – mapping to the  CSDS and OHID MDS – NOA Required child specific data items – mapping to the CSDS sections.

The data items below aim to collect the information required to answer the NOA’s seven questions. In order to limit burden on providers and to maximise the quality of data returns we will seek to collect the minimum amount of data required to ensure the NOA meets its stated aims.

CYP000 Header

A single CYP000 header record is mandatory in every submission file.

All data items in this group are mandatory; the submission file will be rejected if any data items are not populated.

DATA SET VERSION NUMBER

This may be auto-generated by your IT system when preparing your submission file and should be stated as "1.6".

ORGANISATION IDENTIFIER (CODE OF PROVIDER)

This may be auto-generated by your IT system when preparing your submission file.  The organisation code of the weight management provider as issued by the Organisation Data Service (ODS).

Service providers are required to collect the data alongside an official Organisation Data Service code.

ORGANISATION IDENTIFIER (CODE OF SUBMITTING ORGANISATION)

This may be auto-generated by your IT system when preparing your submission file.  Can be used where a different organisation has submitted on behalf of the healthcare provider.

Where the healthcare provider is the submitting organisation, this should contain the same value as the ORGANISATION IDENTIFIER (CODE OF PROVIDER).

REPORTING PERIOD START DATE

This may be auto-generated by your IT system when preparing your submission file.

The reporting period start date to which this file refers. for example, for the January 2023 reporting period, this would be: 2023-01-01. 

REPORTING PERIOD END DATE

This may be auto-generated by your IT system when preparing your submission file.

The reporting period end date to which this file refers. for example, for the January 2023 reporting period, this would be: 2023-01-01.

DATA AND TIME DATES SET CREATED

This may be auto-generated by your IT system when preparing your submission file.

Date/time this submission file was created for example 2023-01-15T10:15:00. 

PRIMARY DATA COLLECTION SYSTEM IN USE

This is a free text field and may be auto-generated by your IT system when preparing your submission file for example TPP SystmOne, Rio, EMIS Web, HICOM.

Where multiple systems are in use, please include the primary system in use from which the highest number of records is extracted.
