This section should be filled in only for:

  • for patients receiving care from a Tier 3 weight management service (adults’ or children’s).
  • for children receiving care within a school nursing Tier 2 weight management service or an evidence based intervention aimed at offering referral to a Tier 2 weight management service.

The member of staff providing each care contact should be recorded whether or not the individual attends.

This table should include one record for every Health Professional (responsible for providing the patients care) and any other staff member who has a contact with a patient.

Where a member of staff has multiple roles or works in more than one team concurrently, a separate record with different Care Professional Local Identifier should be created to enable linkage to activity.

General table guidance

The Care Professional Local Identifier has been included in the CYP202 Care Activity table to allow reporting of all activity by job role and main specialty.


CARE PROFESSIONAL LOCAL IDENTIFIER

A unique local CARE PROFESSIONAL IDENTIFIER within a Health Care Provider and may be assigned automatically by the computer system.


CARE PROFESSIONAL STAFF GROUP (COMMUNITY CARE)

The staff group of a CARE PROFESSIONAL working in a Community Health Service.


These data items are not required for the NOA.
