Every CYP101 referral record must have an associated CYP001 patient record included in a submission file.


SERVICE REQUEST IDENTIFIER

This is a mandated data item; the record will be rejected if it is not included within this group.

An identifier used to identify a referral uniquely within a health care provider.

This is the primary key that enables groups of data to be joined together.

Where multiple systems are used it is acceptable to include a prefix to the Service Request Identifier, which relates to the system. The prefix enables each identifier to remain truly unique for all submissions from an organisation.

Duplicate Service Request Identifiers within the CYP101 group will cause all associated records to be rejected.


LOCAL PATIENT IDENTIFIER (EXTENDED)

This is a mandated data item; the record will be rejected if it is not included within this group.

This data item is used for linkage back to the CYP001 Master Patient Index and Risk Indicators group.


ORGANISATION IDENTIFIER (CODE OF COMMISSIONER)

This is a mandated data item; the record will be rejected if it is not included within this group.

This is the organisation identifier of the organisation that is commissioning health care provided under this referral or referral type. This can be used to identify a specialist commissioner.


REFERRAL REQUEST RECEIVED DATE

This is a mandated data item; the record will be rejected if it is not included within this group.

This is the date the referral request was received by the Health Care Provider.


REFERRAL REQUEST RECEIVED TIME

This data item is not required for the NOA.


NHS SERVICE AGREEMENT LINE IDENTIFIER

This is a 20 character free text field in CSDS. See NHS Data Model & Dictionary definition of this data item:

For Tier 2 WMS providers, the NOA will recognise submission of the following values:

1 - 2022-23 adult weight management grant funding

2 - Existing public health grant funding

3 - Not known

This is not relevant for tier 3 WMS.


SOURCE OF REFERRAL FOR COMMUNITY

A  classification which identifies the source of referral to a Community Health Service.

Internal referrals should normally be recorded as 'Community Service' and the Referring Organisation Identifier will be the same as the Organisation Identifier (Code of Provider).

For example, for an internal referral the national code to use is 07 (as outlined within the CSDS Enhanced Technical  Output Specification (ETOS)), for Community Health Service (same or other Health Care Provider).

Selected data value notes:

01 – General Medical Practitioner Practice 02 – Self referral

10 – Educational Establishment’ includes schools

13 – Care Home’ includes Care Homes with and without nursing

19 – Telephone or Electronic Access Service’ includes the NHS 111 service

24 – Pharmacy


ORGANISATION IDENTIFIER (REFERRING ORGANISATION)

This field should contain the Organisation Identifier of the referring organisation. For internal referrals the Referring Organisation Identifier will be the same as the Organisation Identifier (Code of Provider). It will not be applicable for a self-referral and the following code should be used: X99998.


REFERRING CARE PROFESSIONAL STAFF GROUP (COMMUNITY CARE)

This data item is not required for the NOA.


PRIORITY TYPE CODE

This data item is not required for the NOA.


PRIMARY REASON FOR REFERRAL (COMMUNITY CARE)

This data item is not required for the NOA.


SERVICE DISCHARGE DATE

The date a patient was discharged from the service.


DISCHARGE LETTER ISSUED DATE (COMMUNITY CARE)

This data item is not required for the NOA.
