Every CYP403 Child Protection Plan record must have an associated CYP001 patient record included in a submission file.  This group can only be submitted for children who were under 19 years of age at the start of the reporting period.

LOCAL PATIENT IDENTIFIER (EXTENDED)

This is a mandated data item; the record will be rejected if it is not included within this group. This data item is used for linkage back to the CYP001 Master Patient Index and Risk Indicators group.

CHILD PROTECTION PLAN REASON CODE

The reason the child or young person is subject to an active Child Protection Plan.

If a provider is made aware that the CHILD PROTECTION PLAN REASON CODE has been updated in a given month, this information should still be recorded and flowed even if the provider has not seen the child during that month.

CHILD PROTECTION PLAN START DATE

The date on which a child or young person is placed on a Child Protection Plan.

CHILD PROTECTION PLAN END DATE

The date on which a child or young person is removed from a Child Protection Plan.
