5. Age of patient making complaint
For new complaints only, enter the number of complaints received by the age of patients by or on behalf of whom the complaint is made. Only positive whole numbers should be entered. Any age category left blank will be counted as a zero (0) response.
The total complaints by age box will be completed automatically. Make sure the sum of the individual boxes matches your total of complaints by patient age.
The total complaints by age box must also equal the total number of new complaints received in 2025-26 as in Step 2 complaints received or you will be unable to continue.
