How Confirm your home address works

How this service helps patients confirm their address and supports accurate NHS records and GP practice lists.

Overview

Confirm your home address helps NHS England maintain accurate patient address information and support GP patient list management.

The service identifies patients whose address information may need checking and asks them to confirm whether the address held on their NHS record is still correct.

What happens when a patient responds

The action taken depends on the patient's response:

The patient confirms their address

If the patient confirms they still live at their current address:

they remain registered with their current GP surgery
their address is treated as confirmed
they are removed from NHS address verification activity

The patient has moved within England

If the patient has moved to a new address in England:

they are directed to update their address details
the service checks whether the new address is within their GP surgery's catchment area

If the patient has moved outside their GP surgery's catchment area, they may be advised to contact their GP surgery or register with another practice.

The patient has moved elsewhere in the UK

If the patient has moved outside England to another part of the UK, they are removed from their current GP surgery list within 30 days.

The patient has moved outside the UK

If the patient confirms they have moved outside the UK they:

can report this through the service
are removed from their GP surgery list and NHS registration records as appropriate

This helps ensure NHS records and GP practice lists remain accurate and up to date.

How this service works with other services

NHS login	Patients must have an NHS account with a fully verified identity before they can use the online service.
Personal Demographics Service (PDS)	PDS stores patient demographic information, including address details and address verification flags.
