Quick guide to submitting Restrictive Interventions data for the Adult Acute and Adult Learning Disabilities CQUIN

This is a quick guide to help data submitters understand the tables and data items required for the Restrictive Interventions Adult Acute and Adult Learning Disabilities CQUIN. This guide was updated in May-24 to reflect the implementation of MHSDS v6.

Aim

NHS England and other arm’s length bodies are working to improve the recording and reporting of Restrictive Interventions data in Adult Acute and Adult Learning Disabilities hospital settings using the Mental Health Services Data Set (MHSDS).

CQUIN Reports

View reporting on the data quality of restrictive interventions in Adult Acute and Adult Learning Disabilities settings.

Please note that with the implementation of MHSDS v6 - it is now mandatory to submit an MHS515RestrictiveinterventionType record for each RESTRICTIVE INTERVENTION INCIDENT IDENTIFIER recorded in MHS505RestrictiveInterventInc.

Therefore any RESTRICTIVE INTERVENTION INCIDENT IDENTIFIER recorded in MHS505RestrictiveInterventInc without a corresponding MHS515RestrictiveinterventionType record in the same submission will be rejected from the dataset.


Mandatory tables needed for all MHSDS submissions

The following 4 tables need to be submitted in each submission with their respective mandatory and required fields. More information can be found in the MHSDS User Guidance and Technical Output Specification.

Table Description
MHS000Header Uniquely identifies each MHSDS submission made by the provider.
MHS001MPI Records personal details of each patient.
MHS002GP Records details of the GP of every patient.
MHS101Referral Records details of referrals made for the patient.

Hospital table submissions needed for the Adult Acute and Adult Learning Disabilities Restrictive Intervention CQUIN

Table Description Importance to CQUIN
MHS501HospProvSpell Records details of each Hospital Provider Spell for a patient. Submissions made to other 'Hospital Spell' tables will be rejected without this table.
MHS502WardStay Records details of Ward Stays which occurred during a Hospital Provider Spell for the patient. Is used to record details about the patients ward stay.
MHS505RestrictiveInterventInc Records details of each separate reported incident during a Hospital Provider Spell.
 

Is used to identify:

  • why a patient was restrained
  • when a patient was restrained
  • how long a patient was restrained
  • whether a post-incident review was held for both the patient and care-personnel
MHS515RestrictiveInterventType Records details of each separate reported type of a Restrictive Intervention used, as part of a Restrictive Intervention Incident.

Is used to identify:

  • each type of restraint used on a patient during a Restrictive Intervention Incident
  • when a type of restraint was used on a patient during a Restrictive Intervention Incident
  • how long each type of restraint was used on a patient during a Restrictive Intervention Incident
  • whether the patient, care-personnel or any other person was injured during the application of a Restrictive Intervention type used on a patient during a Restrictive Intervention Incident
MHS903WardDetails Records the details of the ward where patients may be treated during a Hospital Provider Spell. Is used to record details about the ward the patient was treated on.

Submissions in scope of the CQUIN

Submissions to Restrictive Intervention tables in scope of the CQUIN will be identified using the MHAdmittedPatientClass field in the MHS502WardStay table and only include ward stays where this field was recorded as:

Data Items used to calculate the MHSDS RI Data Quality score

Only required fields will contribute to the MHSDS RI Data Quality score. Mandatory fields will not contribute to the score but still need to be submitted for each record - otherwise the record will be rejected.

The table below lists all the MHSDS required fields used by NHS England to derive the MHSDS RI Data Quality score.

An indication of whether a Restrictive Intervention Post-Incident Review for Care Personnel was held within 24 hours of the incident of a Restrictive Intervention.

  RestrictiveIntReason The reason that a Restrictive Intervention was used on a PATIENT during a Hospital Provider Spell.
MHS515RestrictiveInterventType StartTimeRestrictiveIntType The Start Time of the Restrictive Intervention Type as reported by the Care Professional.
  RestrictiveIntType Type of RESTRICTIVE INTERVENTION used.
  EndDateRestrictiveIntType The End Date of the Restrictive Intervention Type as reported by the Care Professional.
  EndTimeRestrictiveIntType The End Time of the Restrictive Intervention Type as reported by the Care Professional.
  RestraintInjuryPatient An indication of whether an injury was sustained to the PATIENT during an incident of restraint during a Restrictive Intervention.
  RestraintInjuryCarePers An indication of whether an injury was sustained to a member of Care Personnel during an incident of restraint during a Restrictive Intervention.
  RestraintInjuryOtherPers An indication of whether an injury was sustained to a person, other than the PATIENT or Care Personnel, during an incident of restraint during a Restrictive Intervention.
MHS903WardDetails SiteIDOfWard The Organisation Site where the patient was treated.

Calculating the CQUIN Score

Only Restrictive Intervention Incident Identifiers that appear in the MHS505RestrictiveInterventInc and the MHS515RestrictiveInterventType tables submitted in the same reporting period will be used to assess data quality.

Meaning that if a provider does not populate the MHS515RestrictiveInterventType table at all - then they will not appear in the report.

The report field ‘MHSDS RI Data Quality Score Adjusted for Coverage’ should be used to assess performance.

Please note that for reports from April 2024 onwards - the MHSDS RI Data Quality Score Adjusted for Coverage = MHSDS RI Data Quality Score.

This is because from April-24 onwards - all RESTRICTIVE INTERVENTION INCIDENT IDENTIFIER recorded in MHS505RestrictiveInterventInc must also have a RESTRICTIVE INTERVENTION INCIDENT IDENTIFIER recorded in MHS515RestrictiveInterventType.

MHSDS RI Data Quality Score Adjusted for Coverage = MHSDS RI Data Quality Score x Coverage

MHSDS RI Data Quality Score = percentage of records valid and complete aggregated across all assessed columns

Coverage = % of MHS505 Restrictive Intervention Incident identifiers also present in the MHS515 Restrictive Intervention Type tables
