This integration is retired and not available for use - use Transfer of Care FHIR instead. Depending on your use case this is one of Acute Inpatient Discharge, Emergency Care Discharge or Mental Health Discharge.

Use this integration to send a discharge report following transfer of responsibility for a patient, such as from an acute hospital, emergency care or mental health care to a GP practice. It contains a summary of care events and care data.
