Data Quality Issue
Due to a processing issue, the originally published values for PCAs associated with indicator CHOL003 were incorrect; patients who were removed from the indicator on account of the PCA regarding non-response to two invitations for lipid monitoring were not included in the PCA count. The issue affected 2,579 of the 6,145 practices included in the dataset, with PCA values for CHOL003 reduced by 3.2 percentage points for these practices as a result.
This issue has now been rectified, and updated versions of the following files (labelled 'v2' in the filenames) are now available below:

- Prevalence, achievement and personalised care adjustments at ICB level
- Prevalence, achievement and personalised care adjustments at sub-ICB location level

Summary
The objective of the Quality and Outcomes Framework (QOF) is to improve the care patients are given by rewarding practices for the quality of care they provide to their patients, based on several indicators across a range of key areas of clinical care and public health. This publication provides data for the reporting year 1 April 2025 to 31 March 2026 and covers all General Practices in England that participated in QOF in 2025-26.
GP practices have been mapped to their respective Sub ICB Locations, ICBs and Regions using reference data current on 1 April 2026. This mapping has been applied to data for both the current and previous reporting year.

Participation in QOF is voluntary, though participation rates are very high, with coverage of 98.3% this year
QOF recorded prevalence rates
Hypertension (15.8%), depression (14.9%) and obesity (14.3%) had the highest QOF recorded prevalence rates.
Changes in QOF recorded prevalence
Non-diabetic hyperglycaemia had the greatest change in QOF recorded prevalence, which was 0.7 percentage points higher than in 2024-25.
Practice achievement
4,660 (75.8%) of practices achieved over 90% of the available QOF points.
Value of a QOF point
A QOF point had a value of £225.49, an increase of 2.2% compared to 2024-25
Personalised care adjustments rates
The chronic obstructive pulmonary disease indicator group had the highest personalised care adjustment (PCA) rate at 28.5%. Smoking had the lowest PCA rate at 1.8%.
