Opt out of the Children's Surgery Outcome Reporting (CSOR) programme

Use this form if you do not want your child to be invited to take part in the CSOR programme.

You can request not to receive an invitation

If you've previously opted out of sharing your health data for planning and research purposes, by submitting a National Data Opt-out, you do not need to take any action. You will not be invited to take part in research by NHS England.

For everyone else, you can submit the form below if you do not want your child to be invited to take part in the CSOR programme.

Enter your child's date of birth as DD/MM/YYYY for example 01/02/2020.
You must enter the home postcode you registered with your GP surgery and include a space in the middle. For example EN1 1BE.
