Module 3.8 - Incident management and monitoring
This module addresses reactive clinical risk management — incident reporting/recording, investigation, change control and post-deployment monitoring — and how learning from patient safety incidents feeds back into controls, training and the safety case.
This module will provide:
- Incident management, change control and post-deployment monitoring
- How patient safety events are recorded and responded to for the purpose of learning and improvement
- Understanding of how organisations learn from patient safety events, such as Patient Safety Incident Response Framework (PSIRF)
- Integrates incidents and monitoring into CRM artefacts
- Analyses incident themes and translates findings into action
- Vigilant and responsive throughout system life
