Safeguarding Case Review Tracker (S-CRT)

Background to the collection

This data collection will support the comprehensive case tracking of Serious Incidents arising due to a safeguarding failure. In broad terms, Serious Incidents are events in health care where the potential for learning is so great, or the consequences to patients, families and carers, staff or organisations are so significant, that they warrant using additional resources to mount a comprehensive response.

Serious Incidents can extend beyond situations which affect patients directly and include incidents which may indirectly impact patient safety or an organisation’s ability to deliver ongoing healthcare.

NHS Digital will provide a Safeguarding Case Review Tracker that will enable the collection and analysis of information about Serious Incidents that have resulted from safeguarding failures. The data will be submitted into the Safeguarding Case Review Tracker by Clinical Commissioning Groups (CCGs) in England.


Benefits of the collection

The data collected will enable CCGs to identify, track, report and monitor the number of Serious Incidents that have occurred due to a safeguarding failure, and to record and capture lessons and outcomes from safeguarding reviews. NHS Digital will collate the data from the Safeguarding Case Review Tracker and disseminate this to NHS England and NHS Improvement (NHSE/I) which will support:

  • NHSE/I to successfully deliver its safeguarding statutory duties, roles and responsibilities as set out in the ‘Safeguarding children, young people and adults at risk in the NHS: Safeguarding accountability and assurance framework’
  • NHSE/I to identify and report on the number of Safeguarding Multi-Agency Reviews, the causes, themes, and the learning that have occurred at the end of each month, quarterly and annually, as part of wider patient safety and quality of care reporting
  • monitoring and tracking of case reviews, which will provide the Quality and Safeguarding team with the thematic views of vulnerability, safeguarding and emerging challenges nationally, the organisational learning, the type of review undertaken and recommendations from the reviews
  • Integrated Care System place-based discussion on live safeguarding case reviews
  • monitoring of reviews to enable the system to understand what is new, open, or closed so providing a current live picture of the safeguarding workload in the system
  • mitigation of risk and will help influence new national safeguarding workstreams and support policy review and development
