‘NHS Continuing Healthcare’ (NHS CHC) is a package of ongoing care that is 100% funded solely by the NHS where the individual has been found to have a ‘primary health need’ as set out in the National Framework for NHS Continuing Healthcare and NHS-funded nursing care. Such care is provided to an individual aged 18 or over, to meet needs that have arisen, as a result of, disability, accident or illness.
The data set includes: number eligible for NHS CHC, incomplete referrals over 28 days, number of new referrals, number of referrals completed, number of referrals completed within 28 days, number of assessments completed, number assessed as eligible/ not eligible, number no longer eligible, assessment conversion rate, referral conversion rate, number of requests for a review of a sub Integrated Care Board (ICB) location eligibility decision (and review outcome), previously unassessed periods of care, number of DSTs completed in acute/ non-acute setting and NHS-funded Nursing Care activity.
