PCN Appointments Data Dashboard access request form
Apply for access to the PCN Appointments Data dashboard.
Before you start
Who can apply
You can apply for access if you:
- are a PCN staff member - applications from non-PCN staff will not be accepted
- have one of the required activity codes listed below on your smartcard for at least one practice within your PCN area
Required activity codes
- B0360 (view detailed health records) which can be inherited from B8006 (execute clinical reports)
- B0258 (execute contracting reports)
- B0945 (execute management reports)
- B0560 (perform patient admin)
Your local Registration Authority (RA) can help with adding one of these activity codes. If you do not know your local RA, check our service provider contact details page, or contact your local Clinical Commissioning Group (CCG).
Check your system status
You’ll need to check your system status with regards to NHS Credential Management software which is required to access the dashboards via modern browsers. This is currently being deployed by CCGs and IT support organisations across the NHS estate.
Check if NHS Credential Management is installed on your machine.
If the above link informs you that NHS Credential Management is not installed, we recommend that you contact your local CCG or IT support to find out the deployment path. Deployments are happening rapidly, and your situation will likely be resolved soon.
If this does not solve your immediate issue, discuss options with your local IT on how to get your machine updated ahead of their deployment schedule, or call the NHS Digital service desk on: 0300 303 5678 (9am to 5pm, Monday to Friday, except public holidays).

Personal data collected during this process will only be used to onboard and manage the users of the PCN Appointments Data Dashboard. It will not be used for any other purpose.

ODS (organisation) code
