If you have a vaccine booking query:

If your query relates to personal information that is incorrect on your patient record, such as name or address:

• Contact your GP practice

If you were vaccinated abroad with a vaccine approved for use in the UK, this can now be recorded on the National Immunisation Management System:

• Online with the National Booking Service
• Telephone: 119
