The Register with a GP surgery service is a free digital and paper registration service from NHS England. It allows patients to choose to register in person or online at a time that is convenient to them. This secure centrally managed service gathers patient demographic and medical information in accordance with practice needs and current NHS policy. It uses this information to match citizens to their Personal Demographic Record (PDS). This data is then sent electronically to GP surgeries for the purpose of registration.
Features
The service allows a GP surgery to:
- take registrations in the NHS App and on the NHS website
- check if a patient is in their catchment area
- ask all the questions they need to process a registration
- add optional extra questions
- match patients to their NHS record
- use NHS login to make registration quicker for patients
- tell patients when their registration has been accepted
- download a matching paper form
- work with third party suppliers to automatically enter data into their local systems using Robotic Process Automation
The service complies with:
- WCAG AA accessibility standard
- NHS England Primary Medical Care Policy and Guidance Manual (PGM)
- privacy and security regulations

All GP surgeries in England must offer the national Register with a GP surgery service from October 2024 as outlined in the GP contract.
It is free for NHS GP practices to use.
