ORD changes phase 1 extension questionnaire

Background

The Organisation Reference Data (ORD) maintained by the Organisation Data Service (ODS) had an Information Standard (0090) for 'Health and Social Care Organisation Reference Data approved and published in August 2016. The Information Standard mandates a series of modifications to the existing data to address some long-standing issues.

The ORD Changes project team have created several tools to explain and assess the potential impact of the changes.

Why we are consulting

The purpose of this survey is to get your feedback on the potential impact of the changes described in the Information Standard Notice (ISN).  The results will be used to identify and plan how we can support you with these changes.

We do ask for some personal information (contact details) within the questionnaire, in order to follow up with you if required and offer you relevant advice and guidance. Any identifiable information you provide will be kept securely and will not be published or shared. Your individual responses will only be available to the ORD Project Team.

Participation in the survey is completely voluntary, and it should take no more than 5 minutes to complete. You can withdraw your response at any point whilst the survey remains open.  Should you wish to withdraw your response after the survey has closed, please contact the ORD Project Team (see below for contact details).

Timescales

This is an open ended questionnaire.  The project team will be monitoring responses and following up with additional advice and guidance if required. A summary of key findings from the survey will be published via the ORD Information Portal, at regular intervals.
