Update (November 2012): Error in contextual indicator 'Deaths split by those occurring in hospital and those occurring outside hospital within 30 days of discharge'
A small error has been detected in the SHMI contextual indicator 'Deaths split by those occurring in hospital and those occurring outside hospital within 30 days of discharge'. An issue has been identified with the methodology we are using to identify deaths that occur in hospital and deaths that occur outside hospital within 30 days of discharge.
On the October 2012 release, this figure will be revised to approximately 74.1 per cent and 25.9 per cent respectively. Changes to previous quarters are similar. A revised methodology specification document will be published with the next release in January 2013.
This is the fourth publication of the Summary Hospital-level Mortality Indicator (SHMI). There is a methodological change for this publication. Further details of this change can be found on the methodological changes page (see Related Links).
There are now 143 providers being reported on the SHMI compared to 147 previously. This is due to trust mergers. The details of the mergers including the associated provider codes are below:
• Barts Health NHS Trust (R1H) is a new trust that has been formed from the merger of Barts and The London NHS Trust (RNJ), Newham University Hospital NHS Trust (RNH) and the Whipps Cross University Hospital NHS Trust (RGC).
• Trafford Healthcare NHS Trust (RM4) has now merged with Central Manchester University Hospitals NHS Foundation Trust (RW3) and will be reported under Central Manchester University Hospitals NHS Foundation Trust (RW3) going forwards.
• Winchester and Eastleigh Healthcare NHS Trust (RN1) has now merged with Basingstoke And North Hampshire NHS Foundation Trust (RN5) and will be reported under Basingstoke And North Hampshire NHS Foundation Trust (RN5) going forwards.
Note:
Scarborough and North East Yorkshire Healthcare NHS Trust (RCC) and York Teaching Hospital NHS Foundation Trust (RCB) are still published separately for this publication although they have now consolidated. This is due to the special nature of it not being a merger or direct succession and discussions on how to deal with this were still underway at time of production. The decision to treat this like merger was reached only recently. Therefore, these two providers will be reported under York Teaching Hospital NHS Foundation Trust (RCB) from the next publication onwards.
Key Facts
In the period from 1 January 2011 to 31 December 2011:
• 10 trusts had a SHMI value categorised as 'higher than expected'
• 16 trusts had a SHMI value categorised as 'lower than expected'
• 117 trusts had a SHMI value categorised as 'as expected'
• The percentage of patient admissions with palliative care coded at either diagnosis or specialty level is approximately 1.0 per cent
• The percentage of patient deaths with palliative care coded at either diagnosis or specialty level is approximately 17.2 per cent
• The percentage of elective admissions where a death occurs either in hospital or within thirty days (inclusive) of being discharged is approximately 0.7 per cent
• The percentage of non-elective admissions including admissions coded as 'unknown' where a death occurs either in hospital or within thirty days (inclusive) of being discharged is approximately 3.6 per cent
• The percentage of deaths split by those occurring in hospital and those occurring outside hospital within 30 days of discharge is approximately 76.5 per cent and 23.5 per cent respectively.
