How we use the information (processing activities)

The aims of NHS Continuing Healthcare Patient Level Data Set (NHS CHC PLDS)  project are to (a) develop; (b) bring online; and (c) run as a monthly data collection and publication, a patient-level dataset which will cover all aspects of NHS Continuing Healthcare (NHS CHC) and NHS Funded Care.

NHS Continuing Healthcare means a package of ongoing care that is arranged and funded solely by the NHS where the individual has been assessed and found to have a ‘primary health need’ as set out in this National Framework for NHS Continuing Healthcare and NHS funded Nursing Care  Such care is provided to an individual aged 18 or over, to meet health and associated social care needs that have arisen as a result of disability, accident or illness, as defined by the NHS Continuing Healthcare framework.

This will be an end-to-end data set (from referral to end of service provision) covering initial checklist all the way through the assessment process to the commissioning and monitoring of individual care packages.

Processing of information is key in turning raw data into understandable intelligence on all aspects of performance, patient care and efficiency relating to NHS CHC.

How long the data is kept A maximum of 20 years, or dependent also on continuing validity of Directions or as per the current NHS Digital Records Management Policy, should the retention period change. The data will be appraised before it reaches its maximum retention period in accordance with the NHS Digital Policy and either recommended for secure destruction or an extension of the retention period if there is a justifiable reason.

Is the data subject to decisions made solely by computers? (automated decision making) Consent is not the basis for processing. Option available via National Data Opt out to stop your confidential patient information being used for research and planning.
Where does this data come from? NHS Digital will collect the NHS Continuing Healthcare data from all Clinical Commissioning Groups (CCGs) and Integrated Care Boards (ICBs) that commission NHS funded CHC services in England (and any Commissioning Support Units (CSUs) providing this service on behalf of CCGs).
The legal basis for collecting this data

Direction from NHS England under s.254 of the Health and Social Care Act 2012 to establish and operate a system for the collection and analysis of Continuing Healthcare Patient Level Data.  The Continuing Healthcare Directions 2022 are published on the NHS Digital website.
