User guide for the National Perioperative Data Standard web form

NHS Digital is working to identify, collect and submit national perioperative data from NHS, private providers in England and across the devolved nations to generate UK wide coverage.

Register as a user

You will need a laptop or desktop computer. We recommend to use one the following browsers

  • Google Chrome 
  • Microsoft Edge
  • Safari

Follow the below steps/guidance for set-up.

1. E-mail [email protected] to request access to the webform.

2. We will initiate the set up and authentication process.

3. You will be sent an email with a link to the webform and registration details. 

4. Click register and follow the on screen instructions to create an account. 

5. Once you have registered you will receive an email from NHS Digital to confirm we have set up your account.  You may need to check your junk email if the email does not appear in your inbox. 


Signing in

Open the SDCS web page.

Enter your username and password and click sign in.

Adding a new patient record

When you are using the SDCS portal the webform will time out after 120 minutes of inactivity and all data will be lost.

To avoid this, we recommend users should ensure they press save and continue regularly. 


1. Select relevant organisation from the drop-down list (If you are registered at multiple organisations select the organisation where the activity took place).

2. Under the Submissions menu, select New from the drop-down.

3. Select implantable medical devices from the collections list and click the Create button.

4. On the Patient details page you will need to complete the following 5 mandatory data items 

  • local patient identifier
  • person birth date
  • type of operation
  • operation date
  • operation start time

These mandatory data items enable NHS Digital to link data with electronic data submitted through the MESH gateway. This linkage results in the production of the full procedure record.


The mandatory data items should be populated on the webform and failure to provide these data items will restrict progression onto subsequent pages and submission. An alert will pop up if mandatory data items are omitted.

For example, failure to populate the operation date will result in the prompt shown below:

5. Once all the mandatory data items and any other relevant patient details have been entered, click on save and go to summary button.

Clicking the ODS Code link takes users to an external ODS portal page listing ODS codes for health care organisations.


6. At the summary page click change to navigate to the section you want to submit data for. The sections on the summary page are:

  • patient details                  
  • pre-op
  • surgery
  • revision or removal
  • new device            
  • pelvic floor details

If you are entering device details , then NHS Digital require the following minimum data items to be submitted, in order to uniquely identify the device:


    • Global Location Number (GLN) or manufacturer
    • unique device identifier or catalogue reference number
    • device serial number
    • device lot or batch number
    • quantity (where multiple devices from the same batch/lot are implanted - i.e. where each device does not have a serial number unique to that device)
    • expiry date

7. Once all the relevant data items have been entered in a section, click on either save and go to summary, or save and continue. 


8.   Once all relevant sections have been completed, it is recommended to check the information on the summary page before submitting the data to NHS Digital. Click change if you need to make any alterations or additions.

9. Scroll to the bottom of the page and click submit. Once you have submitted the form the data cannot be viewed or edited. 

10. You will receive a submission reference ID confirming we have received the data.

11. Click back to home to return to the beginning to create another record.

12. Click log out on the top right corner of the page to close the browser and end the session.
