Online tool guide for the National Diabetes Footcare Audit (NDFA)

Before you start

Before you can access the National Diabetes Foot care Audit online tool you will need to register for a Single Sign on account. 

You will need to complete all the fields. 

The email address you use will need to the same email address you have used for the registration form form for your organisation.  You will use this email address when you login 


Logging in

You can log in to the NDFA online tool using the following link

https://clinicalaudit.hscic.gov.uk/

Your user name is the email address you used to create your Single Sign On account.  

When you have logged in select the the National Diabetes Footcare Audit (NDFA) audit from the list available. You will only see a list of clinical audits you are registered to see. 


Adding a new patient

1. To add a new patient to the online tool select Add/search for patient record.  

2. Add the patient NHS number and their date of birth and then submit.

3. The patient record will be created in the online tool for you to complete now or so you can come back at a later date. 

4. To add a new foot ulcer episode select add record


Adding a new episode

1. On the episode screen select the foot treating service within your organisation you are entering the data for. You can add as many treating services as you need to.

2. Select the foot care treating service within your organisation you are entering date for. You can have as many treating services as you need to.  

3. Select the data of first assessment.

4. Select the interval between first presentation and assessment.

5. Select the presenting features and the SINBAD foot index ulcer.

6. Once the information about the first assessment has been completed the record can be submitted and will be available to update at a later date.

7. Use the submit button to save the record so far.


Adding 12 week assessments to a patient record in the footcare audit

1. Use the add/search link to find an existing patient record in the online tool. 

2. Enter the patient's NHS number and date of birth.

3. Select edit record for the episode you are entering data for.  Patients can have multiple episodes or ulcers that have been recorded in the footcare audit. 

4. A new episode is defined when if a patient presents with a foot ulcer having had some time before without having any foot ulcer. 

5. When you go back into a patient record you will be able to see information about who created the record and when it was last updated.

6. If a patient has now left your treatment service then you can select the lost to follow up in Section B outcome. 

7. There is a lost to follow up option at 12 weeks.  

8. If the patient is not lost to follow up at the 12 weeks assessment, please complete the fields and select update to save the information.
