Summary

This service can check for a record of a Civil Death Registration in England, Wales and the Isle of Man on behalf of adoptees and birth relatives to establish if an adoptee or birth relative is recorded as having died. You can also use this service to forward information about hereditary medical conditions to an adoptee's or birth relative's GP.

    Hereditary medical conditions

    This service provides the confidential transfer of information relating to an existing medical condition which is hereditary between the GPs of adoptees and birth relatives. Provided the adoptee or birth relative can be identified from the information available and is currently registered with a GP in England, Wales or the Isle of Man, medical information about existing hereditary conditions may be passed on. It is advisable to use the Death Registrations enquiries service before proceeding with a request to pass on information about hereditary medical conditions.
