Make choices about your Summary Care Record

We help information on you to be shared efficiently with the professionals that care for you through the Summary Care Record system. You can opt out of having a Summary Care Record.

Your Summary Care Record is a short summary of your GP medical records. It tells other health and care staff who care for you about the medicines you take and your allergies. This means they can give you better care if you need health care away from your usual doctor's surgery. Everyone has one by default, but you can decide to opt out. Additional information in the SCR, such as details of long-term conditions, significant medical history, or specific communications needs, is now included as default for patients with an SCR, unless you have previously told the NHS that you did not want this information to be shared.
