DOC@HOME enables the safe and secure remote monitoring of patients in their place of residence. Vital signs and Symptoms are collected by the patient and transferred to a clinical database, from where clinical staff can login and view the data, set threshold alerts, manage alerts, with two way messaging. 

ARTEMUS

ARTEMUS  is a population health intelligence tool which links multiple data sets and runs multiple risk tools to detect scores such as admission risk, eFrailty, EoL and Social isolation. Enables the identification of cohorts, service utilisation and costs, gaps in care. Provides prevalence of LTC’s (Long term conditions), benchmarking, evaluation of interventions, and key metrics for business case development.  

Digital Toolkit

Digital Toolkit  is a package of documents which contain policies, guidance, procedures, training guides, information leaflets and template forms in electronic format to enable an organisation to rapidly implement digital remote monitoring services.

Commercial offer

A rapid procurement pathway to purchase and implement digital services, and implementation support from the London Borough of Bexley.

1. DOC@HOME (Patient Remote Monitoring System) which uses MESH messaging to add patient data into GP EPR system.
2. Artemis (population health management tool) combining primary, secondary and social data sets and linking in telehealth data from remote monitoring telecare system.
3. Digital toolkit a guide which will help instruct implementing remote monitoring into private homes and care homes.
4. A published commercial offer for the rest of the sector.
