Hull social care radar: a case study with Hull City Council and Upstream Health

Improvements in data sharing from social care to health which will ensure that patients are given the most appropriate treatment as early as possible.

Background

Within Hull care professionals are provided with access to the local authority’s case management system (Liquid Logic). This enables care professionals to access and review information - however, this is a reactive process, driven by a patient attendance or a call from a health or social care professional. Often by the time the professional reviews the data it is too late for the information to help the patient.

There is a need for a more proactive approach to care delivery - there are plans for the data from Liquid Logic to be structured in such a way that it sends an alert to care professionals to intervene early where it can add greatest value to the patient. This would also allow preventative care to be given, improve administration and simplify information sharing.

Data sharing from social care to health

This project aims to better share data between social care and health for people with care plans and to move from reactive to proactive intervention through a new way of sharing data.

The Social Care Intervention Radar, built by Upstream Health, will be provided to key health teams and GP practices. The software works with the existing case management system to give professionals a real-time view of their patients in the social care system. This Radar will improve data sharing and enable health and care professionals to intervene early, which will reduce admissions and the demand on health services.

Lessons learned to date

  • to avoid delays, prepare documentation such as agreements and financial processes prior to the project start date
  • funding organisation should manage expectations based on short timelines
  • assess external partners for compliance prior to the start of projects
  • information governance and data protection impact assessment processes can take considerable project time.
