SALT measure STS001

Measure description

Numbers of requests for support received from NEW CLIENTS, broken down by the different sequels to that request.

Tables 1a and 1b - 01/04/2021 – 31/03/2022.

Tables 2a and 2b- 01/10/2020 – 31/03/2022.

Relationships with ASCOF

Data from tables 1a and 1b is used for ASCOF measure 2A (Long term support needs met by admission to residential and nursing care homes per 100,000 population). All clients recorded as requesting support through the ‘Self-funder with depleted funds’ route and whose funding for residential or nursing care is taken on by the local authority will count as new admissions. The sequel should be recorded as ‘Long Term Support (eligible services)’ with a nursing or residential support setting.

The data in tables 2a/2b will help give some indication of what proportion of clients receiving long term support or ST-Max have on a recent occasion received ST-Max, i.e. a return rate and can be used and interpreted alongside ASCOF 2D, derived from STS002a.


SALT measure STS002a

Measure description

Of new clients where the sequel to a request for support was ‘Short term Support to Maximise Independence’ (STS001) a breakdown of what followed the period of short term support.

Relationships with ASCOF

ASCOF measure 2D will use the data from this measure to calculate the outcome of short term services: sequel to service. This could also be used in conjunction with SALT measure STS001 tables 2a and 2b by looking at clients who returned within 6 months to request (and be given) further short term support or long term services.

Data from table 5 is also used for ASCOF measure 2A (Long term support needs met by admission to residential and nursing care homes, per 100,000 population).


SALT measure STS002b

Measure description

Of existing clients who have received ‘Short term Support to Maximise Independence’ a breakdown of what followed the period of short term support.

Client aged 18+ at time of sequels.

Relationship with ASCOF

Data from table 2 and 4 are used for ASCOF measure 2A (Long term support needs met by admission to residential and nursing care homes per 100,000 population).

It is recognised that efforts to re-enable or rehabilitate existing clients may be very different in nature to those programmes set up for potential clients contacting the council for the first time, so data from STS002b is not utilised in ASCOF measure 2D (Outcome of short term services: sequel to service).


SALT measure STS004

Measure description

Proportion of older people (65+) who were still at home 91 days after discharge from hospital into reablement / rehabilitation services.

Period for all tables (unless specified)

Hospital discharges between 01/10/2021 – 31/12/2022 with 91 day follow-up.

Clients aged 65+ only.

Relationship with ASCOF

Data from measure STS004 is used for ASCOF measure 2B (proportion of older people, 65 and over, who were still at home 91 days after discharge from hospital into reablement / rehabilitation services). It is considered in relation to the 2D measure, though these cover different perspectives on rehabilitation and reablement.


SALT measure LTS001a

Measure description

The number of people accessing long term support during the year to 31 March by Primary Support Reason, Age Band, Support Setting and Mechanism of Service Delivery.

01/04/2021 – 31/03/2022.

Age banding

Clients aged 18+ only.

Relationships with ASCOF

Data from SALT measure LTS001a is utilised within ASCOF measures 1E (the proportion of adults with a learning disability in paid employment) and 1G (the proportion of adults with a learning disability who live in their own home or with their family).


SALT measure LTS001b

Measure description

Of the clients in LTS001a, the number of people accessing long term support at the year-end (31 March) by Primary Support Reason, Age Band, Support from Carer, Gender, Ethnicity, Support Setting and Mechanism of Service Delivery.

Clients aged 18+ only.

Relationships with ASCOF

Data from this measure will be used for the denominator of the client measures in ASCOF measure 1C (the proportion of people using social care who receive self-directed support and those receiving direct payments).


SALT measure LTS001c

Measure description

Of the clients in LTS001b, the number of people who have been accessing long term support for more than 12 months at the year-end (31 March). Broken down by Primary Support Reason, Age Band, Support Setting and Mechanism of Service Delivery.

Data from this measure (along with STS001, STS002a and STS002b) will be used to capture the number of new admissions to residential and nursing care, for ASCOF measure 2A (Long term support needs met by admission to residential and nursing care homes per 100,000 population).


SALT measure LTS002a

Measure description

Those clients receiving long term support for more than 12 months at the year-end (LTS001c), for whom an unplanned or planned review of care needs took place during the year and the sequel to that review,

SALT measure LTS002b

Measure description

Those clients receiving long term support for more than 12 months at the year-end (LTS001c), for whom an unplanned or planned review of care needs took place during the year and the sequel to that review,

SALT measure LTS003

Measure description

Carer support provided during the year, broken down by the age of the carer, Primary Support Reason of the client (cared-for) and the type of support provided.
