Diabetes Transformation Metrics (DIAB): RETIRED

The diabetes transformation dashboard is updated quarterly to enable CCGs, STPs, regional and national teams to monitor improvements in outputs and outcomes across the four diabetes treatment and care programme priority areas.

Background

The National Diabetes Treatment and Care Transformation Fund has the following key priorities:

  • improving the achievement of the three NICE recommended treatment targets (HbA1c, cholesterol and blood pressure) and driving down variation between CCGs
  • improving attendance at structured education
  • reducing amputations through improved provision of multidisciplinary footcare teams, and improved timeliness of referrals from primary care for patients with diabetic foot disease; and
  • reducing lengths of stay for diabetic inpatients through improved provision of diabetes inpatient specialist nursing services  

NHS England has invested approximately £80 million during 2017/18 and 2018/19 to support individual CCGs, CCG collaborations and sustainability and transformation partnerships (STPs) to improve the treatment and care of people with diabetes, focused on each of the above interventions.

The Diabetes Transformation Dashboard and Data Collection

The diabetes transformation dashboard is updated quarterly to enable CCGs, STPs, regional and national teams to monitor improvements in outputs and outcomes across the four diabetes treatment and care programme priority areas. The dashboard is to support improvements across all localities, whether or not they are in receipt of transformation funding. The dashboard is shared via the Future NHS Collaboration Platform and colleagues can request access by emailing [email protected].

The dashboard includes nationally collected data from a variety of sources (the National Diabetes Audit (NDA), NCVIN Footcare Profiles etc) as well as a small number of additional metrics which are specific to the priorities funded and for which data are not currently collected sufficiently frequently for monitoring purposes. These additional data items are collected from CCGs via this SDCS collection.
